The Lajes Field Veterinary Clinic is located in
Building T-459. To reach us by phone dial 011-351-295-573-134 or
DSN 535-3134. Our e-mail address is:
65MDG.VeterinaryClinic@lajes.af.mil.
Our staff consists of a military animal care technician and a
NAF receptionist. A military veterinarian visits the island
approximately every 8 weeks. Some of the services that we
provide daily are: microchip identification, vaccinations,
parasite testing and control and routine health care. Additional
services that are provided when a veterinarian is present are
sick-calls, dental cleanings, spays, and neuters. Due to the
absence of a full-time veterinarian our emergency services are
severely limited. The technician can only stabilize the animal
until a civilian veterinarian is contacted. The clinic has a
list of civilian veterinarians to assist you in an emergency.
Please note, the technology we take for granted in the U.S. is
generally not utilized by local veterinarians. Certain
medications and special diets are not easily available; we do
not recommend that you ship pets with chronic problems or
special needs. There might be a possibility of ordering
medications for certain animals. Please call or email to confirm
that the medication in need is available through our facility.

The
Azores follows procedures set up by the Portuguese government.
The veterinary treatment facility does not have power over the
requirements for moving a pet to Lajes. Since the Portuguese
Consular frequently changes the requirements for entering the
country, you should contact the Portuguese Embassy, Consular for
current requirements at:
The Portuguese Embassy, Consular Section
2310 Tracy Place, N.W.
Washington, DC 20008
Phone: 202-332-3007
There
is no quarantine unless you fail to fulfill entry
requirements. In general, both rabies and the distemper
vaccinations are needed. The vaccines cannot be more than 11
months or less than 30 days old. A health certificate is
required, dated within 10 days of departure. The health
certificate must cover the duration of your flight to Lajes and
not expire until after you pass through Customs. The bilingual
Portuguese health certificate is located in our FORMS section.
An ISO standard microchip is also required. The FAVN Rabies Anti-boy test is
not required at this moment although it could change. Check with
the Portuguese Embassy for updates. More detailed information

Follow the vaccination requirements above. Check with the
Portuguese Embassy in the country of origin to determine if an
import permit is required. In general, members of the European
Union do not require this permit.

All
animals arriving on the island must be registered with the
veterinary clinic within 72 hours. A microchip identification
system is available and mandatory. Dogs and cats are required to
be immunized against rabies and distemper annually. A
Veterinarian Treatment Facility Policy Form must also be
completed during registration. The VTF Policy Form is located in
our FORMS section.
We
at the Veterinary Treatment Facility hope this information is
useful to you and sincerely welcome you to the Lajes Field
community. If you have further questions, please contact us.
